The Wellness Center Massage Therapy Program provides a calm, relaxing environment focused on physical and mental well-being. Restorative and regular massages offer many benefits for anyone regardless of age or activity level.

Post-Acute Care helps to ease the transition between Home Health, Supportive Care, and Hospice. HCM provides these services to help individuals to continue living at home, improve the patient’s optimal level of well-being, and assist the patient at home. These services help to minimize hospitalization or admission to long-term care institutions. HCM stands ready to care for you and your loved ones at home; because home is where we all want to be.

Home Care

Home Care helps individuals to improve function and live with greater independence; to promote the patient’s optimal level of well-being; and to assist the patient to remain at home, avoiding hospitalization or admission to long-term care institutions.

Supportive Care

Supportive Care helps patients with chronic or serious illnesses improve their quality of life. Supportive care does not focus on fixing the underlying condition but instead focuses on providing relief from symptoms and stress related to serious or chronic illness.

Supportive Care is for patients still wanting aggressive treatment for their condition.

Hospice

Hospice care focuses on the care, comfort, and quality of life of a person with a serious illness who is approaching the end of life.

Hospice care is for patients who are no longer wanting aggressive treatment for their conditions.

Examples of services provided

  • Supportive care assists with advanced care planning and other difficult decisions.
  • Prescribe treatments to manage symptoms (including but not limited to pain, nausea, shortness of breath, anxiety and insomnia) of chronic diseases Ordering labs/imaging.
  • Provides education on disease progression.
  • Coordinates care with other doctors and healthcare providers.
  • Consults at your place of residence
  • A nurse practitioner can order services from home health as needed for a limited time period

Examples of when you might choose Supportive Care

You or a loved one may choose Supportive Care if a serious illness results in difficult symptoms, such as:

  • Uncontrolled pain
  • Nausea
  • Anxiety
  • Shortness of breath
  • Fatigue
  • Delirium
  • Constipation
  • Emotional distress

Supportive Care Goals

  • Provide expert symptom management
  • Improve the patient’s and family’s quality of life
  • Assist patients and their families in better understanding their illness and treatment options

  • Reduce the number of hospital and emergency room visits
  • Facilitate open discussions with patients and families regarding goals of care, concerns and wishes
  • Help in navigating the healthcare system

  • Collaborate with the current medical team to develope a comprehensive plan of care

Supportive care accepts Medicare along with most commercial insurances. HCM is available to guide you to the best care for you or your loved one and answer any questions.

HCM’s supportive care team makes routine visits M-F 8am-5pm.
No on-call provider at this time. Patients will be instructed to call their PCP or go to the Urgent Care/ER

Newborn Care and Prepared Childbirth Class 

The Hill Country Memorial OB Team leads a weekly class at the Cailloux Birthing Center (HCM Women’s Pavilion) on infant care techniques such as diaper changing, bathing, safe sleeping practices, cord care, and identifying signs of jaundice for new parents and caretakers. Both moms and dads—understand the process of labor and delivery and learn techniques to cope with labor.

Breastfeeding Classes
If you are expecting or recently gave birth, check out breastfeeding classes from the Wellness Center at Hill Country Memorial.

Taught by an expert breastfeeding educator, you will learn to master the basics of this simple yet essential task. After this class, you will know anatomy, techniques, and positioning to help the infant latch on, how to prevent problems, how to hand express, and how to properly use a breast pump.

Don’t want to come alone? Bring your spouse. They are more than welcome to join.

Welcome to the Hill Country Memorial Wellness Center. We promote exercise, fitness, and nutrition to help our community live longer and healthier lives.

Our services cover all aspects of health from fitness, training, and relaxation to emergency CPR and First Aid. We also provide classes on nutrition and smoking cessation.

The HCM Wellness Center is home to a large variety of events throughout the year. During the summer months, we host Kids Fitness Camps and youth swimming lessons.
